MDOT Aggregate Source Identification Request (ASI) Form Update

MAA has been informed that the Michigan Department of Transportation (MDOT) has updated their Aggregate Source Identification Request (ASI) Form.
 
The only change to the form is the email address listed at the bottom for submitting requests. Effective immediately, all ASI requests should be sent directly to MDOT’s Aggregate Quality Unit for processing rather than to the AASHTOWare database team.
 
MDOT recommends that members verify they are using the most current version of any form prior to submission by checking the official MDOT forms repository.
